Death Registration
To register a death in the City of Cornwall, you must submit the following documents:

Completed Statement of Death

Medical Certificate of Death or Warrant to Bury the Body of a Deceased Person
You can download the required forms from the Government of Ontario. You can also submit your documents by fax 613-933-1860 or drop them off in the secure mail box at the front door of City Hall at 360 Pitt St.
Request a copy of a Death Certificate
You can apply for a death certificate through ServiceOntario. You can also pick up an application form in person at our Vital Statistics Office at 360 Pitt St.
